BridgeWell Health Services was founded by an ER doctor, a former nurse, and a psychiatric nurse practitioner with a shared vision: to provide a safe, seamless, and compassionate path for patients returning home from hospitals, rehab facilities, and skilled nursing centers.
We saw how many patients struggled during care transitions. We built BridgeWell to close that gap—with structure, compassion, and clinical excellence.
